Please use the Announcement Request Form to guide you through the communication efforts for your event. The information collected with this form will be used to design an announcement graphic. This graphic will then be displayed in the requested locations during the requested time frames.

*All events must be approved by a supervising staff member and on the CCTV office calendar before the form is submitted.
